Doc to Form allows you to quickly and easily create a Google Form from within a Doc. Simply select text and click a button to add your questions. You can choose from a variety of question types - ideal for converting traditional worksheets to Google Forms.

Loved it! I can outline and develop my form in Word, and then easily create form from there.

This was quite easy to use. For a checklist: Start with a question (which can be a statement or title for an upcoming series of check boxes) Then add the content you want to have check boxes to the left of to the options section. You'll end up with a nice checklist with the "question" at the top as the header. A donation is requested if you need over 10 questions. (donationware model) Very excellent! I recommend you reward this author for his hard work whether you exceed ten questions or not! :-)

I've been waiting for a developer to create a TXT file to Form tool to solve the issue of adding questions quickly, but this has exceeded my expectations. Well done.
