Overview
This app makes the process of creating Forms much quicker. It allows users to enter all the questions and other details using a Google Spreadsheet and then press a button to create the form. It supports all the question types (except form upload) and allows users to set validation rules. Users can make single forms from individual tabs or many forms from 1 tab

Form Maker will allow you to create forms, including quizzes, using the data in a spreadsheet. It will make the process of creating forms very much quicker and easier

It will cope with all the question types available in forms and allows you to add images and videos. 

You can also edit and update questions from the spreadsheet.

add validation rules, and add feedback for quiz questions.

setup a question bank and select subsets of those questions to appear in your form. 

import questions from an existing form to use in a new form and add existing questions to your question bank.  

add Navigation rules to control which page appears next depending on the user's answers to multiple choice and drop-down list questions.

create options lists of up to 1,000 items from the data in your spreadsheet.

Make the options lists dynamic, updating hourly or on form submit.

Preserve form branching rules when options lists are automatically updated.

You can also use it to retrieve location information about images uploaded through a form. It will give you latitude, longitude and address data (if it was captured when the image was taken). It will also include a thumbnail of the image in your spreadsheet.

Looked promising, BUT it did not work. Creating a new sheet never ever ended... :-(

Fine that's it's a paid add-on. Garbage Martin has set the trial limits so low there is no possible way to see if it works without buying it. Don't waste your time. The 10 day evaluation is fake.

This solution is a great solution for those who need to develop surveys with 10+ questions or have a data spreadsheet you'd like to use to create questions. Great job!

You should be honest in your description and tell people clearly that this is only a trial version and they will have to pay to use this add on after 10 days.

Hi Martin, really like the add-on. It sure can prove to be very helpful here. As stated: tutorials are very much needed and one important question for us: is there a possibility to jump to a specific section based on an answer. We have a very complicated form with over 176 sections and would really help us if we would be able to link them to Google Sheets... so your add-on would come in really handy

A good tool to save time, especially as a teacher and with structurally identical, regularly reoccurring testing formats. The one big thing I am craving, though, is the option of adding a "correct answer" to the question, especially in short-text questions, because I often use Google Forms for vocabulary quizzes and the like. Could you integrate an option like that? It would make a good productivity tool into a brilliant productivity tool! :-)

Hi Johannes, I'm pleased you are finding it useful. Re the query about adding "correct answers" to short text questions, it is not yet possible through the API when Google make it available I'll add it in. In the meantime, for those questions, I add the correct answer in the feedback column. That makes it a bit quicker to add the correct answer when I edit the question in the forms editor. Initial impression is that this is exactly what we need! Two questions: it seems like when you do a dropdown without "correct" answers, the form creation hangs. If you put all the answers in the "correct" column it works fine, but may want to make that clear so people like me aren't confused. Second question: Question bank is awesome! However, we want to have section headers and pick, say, 2 questions from each section randomly for the form generation. We'd like to have the form generation pull the section header and all the questions needed per section at once. Is there any simple way to do this?
