Overview
Do you want to create a Form directly from questions in your Spreadsheet? FormCreator is your jam then!

FormCreator is the original Add-on to make a Form from a Sheet, published back in 2014. 

Create a Form, even a Quiz with the correct points and feedback, directly from the values in your spreadsheet. FormCreator builds the template needed to create the Form. 

Populate the template with your questions, check the questions you would like to use in the Form, and then Create your Form!
Works great with FormRecycler, bit.ly/FormRecycler, that lets you easily reuse questions from a previously created Form.

You can also create multiple versions of a Form with a specified number of questions. Just choose the questions you want to use with the checkboxes and then Create Multiple Forms. FormCreator does the rest!

Version 42: Updated with Create Multiple Forms feature.

Version 41: Updated to include 'Other' option for Multiple Choice questions. Just type the word 'Other' exactly (without the apostrophes) in the multiple choice option column for that entry.

What happened? Now all of my spreadsheets from last year won't load! I just lost literally thousands of test banks in one go!!!!!

The spreadsheets are not lost, the format of creating has changed with the updated features. You can use =Transpose to turn horizontal choices into vertical ones.

Hi! First, I love this tool very much--I'm just starting to play around with it and I don't have it working yet, I have but thank you so much for creating this! I have one question: 1. I stupidly bought an account on my personal Google account and NOT my work one, which is what I need. Would it be too much to ask for you to switch it over for me? The add on is a nice template that can reduce hours of copy and paste redundancy. Thanks John! The one request I would make would be to add another column into the sheet for Weight so that one can assign a weighted value for the question answer options. For example, if there are 3 options, ABC I may want to assign A with a value of 2, B with a value of 3 and C with a value of 4. I assume it's a light tweak, but I think it'd be helpful to most users to have this option.---thanks!

This is very helpful once one works out how to use it. There is very little documentation. I think that we the users should attempt to provide such documentation. Now that I have just about got the hang of using it, it makes creating online surveys far easier than creating each question one by one in Google forms. At first I was unable to use this tool at all because I was unable to paste questions into the form. I managed to work out how to paste into the worsheet created by FormCreator. It seems that something in Firefox was preventing my doing this. Once I use Chrome I have no difficulty. This was not a FormCreator issue.

Hi, may I know if it is possible to set one of the answer in the multiple choice options as the correct answer that would be graded automatically?

I love this! It works like a champ for some question types, but I would very much like to use it with a Linear Scale response and when I put in that question type the minions never complete their work and the wheel just spins. I believe it has to do with that question type. Is there a remedy? Have I missed something?

This doesn't seem to be suitable for quizzes with only one correct answer. If I'm going with 'Short Text' as my answer type in GForms, once the import is complete, I still have to type in all the answers into GForms. This template doesn't have the option to add specific answers to the GSheet.
