This add-on will allow you to easily send the results of a form submission via email to people you have listed and/or the person completing the form. In that email you can include the form results, link to the summary of results, and even a link to edit the responses.

~ Merged document (Document or PDF)

Great add on, only one small formatting problem: All the responses seem to come in 1 line. Even after adding HTML Formatting, all responses are not separated by a line break.

Like many others, I have discovered that the app has ceased working. On further investigation I found that the app had been updated and requires new permissions before it will operate again. I am not against improvements, but for it to stop operating without warning is poor. I searched my emails and no warning of the result of the update was disseminated there either. Good app, but if it is unreliable like this again I will be swapping to a competitor.

Great Add-on, thank you! We are missing, that sender's e-mail name and address is dependent on who actually filled out the form. Will this be available soon?

Unfortunately, it will not be an option. Google doesn't allow developers to change the "From" address. There is a method for changing the "replyTo" address, which I played with, but trying to explain the difference between from and replyto, got too cumbersum, and I really want to keep this add-on as "simple" as possible.

This is not working, I can't get it to send automatically or when I tell it to send last. Very dissapointed as it seems none of these apps are working now.

Those of you reporting an error, please do so using the G+ Community. I do not get notified when you post here, and I usually respond almost immediately when people post to the g+ Community. I was unaware that there was an issue, and I have not made any changes that would have caused it to shut off. I think this is on Google's end, but I will test.

Same. This add-on has become an important feature to how I am using several forms. It seems to have stopped working. Can we get a quick fix for this?

The add on does not work. I do not receive notifications. Also the "resend last submission" does not work. It keeps saying "sending" but never sends the notification.

Mr. Developer, this seems like a serious flaw that should be responded to.
me too I keep missing submissions. Even after re-setting it still won't turn on again?

I really do like it. I just have one problem. When I set up a grid question for users, I can get the "Simply Send" to send the email, but it doesn't show the text I put as the labels. It just shows "column 1 and row 1". Is there a way to change so the email shows the text associated with each option?

I am having trouble getting it to merge the responses. Where do I find a list of the field names so I can indicate where I want it in my merge document?

I have tried to figure out a way to make it show the row labels, but I have not found a way, so right now the "questionTitle[row#] = column label.
